Please fill out the following packet to register your child for the 2022-2023 school year. You must download the packet before you can edit it online with your information. After filling out the packet, you will need to print it and bring it with you to the school to complete the registration process. You may also print the packet and complete it with a black or blue ink pen.

 

If you do not have a way to print the forms, you may choose to complete the packet at the school. 

 

You must also bring the child's -- birth certificate, social security card, and proof of residency when you come.
